utility cover missing

A utility lid has become dislodged and it is reseated upside down. It is stuck securely in this position, and it presents a road hazard to drivers. I was not able to dislodge it and reseat it myself, as it is firmly stuck in this position.

There is a utility cover missing from 12th & Monroe. Directly across street from address I gave. I fell in street because of it a couple months ago. A neighbor told me it was off for months already at that time and she reported it previously. Crews have worked in area and it still remains off. I suggest you cover this gaping hole. I hurt my ankle and knee when I was crossing street due to this on February 4. Nobody expects there to be a hole like this in the road. And it's dark
there at night and generally people are looking for oncoming traffic. I'm fortunate that there was none when I fell. I could have been injured much worse if hit by a car. My ankle and knee swell sometimes and hurt intermittently but seem to be slowly healing. The city is negligent if this remains off for much longer or anybody else gets hurt because of it since you have been properly notified at least once (me) probably on another occasion (neighbor).

Metal utility box is open and wires exposed

This gas cover is loose and jumps out of the hole to shred tires and damage cars. We have seen 2 cars damaged in the last few weeks from this cover. Sometimes it sits at an angle on the road and other times it is flush, but still jumps out.

The work done this past week shook our house so much now our water heater was shook loose. Now we do not have hot water. This shook the house so bad that it was shaking out of control for hours on Friday the 4th and Thursday the 3rd of August.This is not our responsibility and the city should come and reconnect properly. utility cover on moses lane is damaged leading to an open hole in the road

Medal meter cover sticking up above the road surface on Taylor Pl NW & Maple Ave NW. The medal meter cover is much higher than the road. One could seriously damage their vehicle if they struck it while driving.
